Conference Objectives

Upon completion, participants will be able to:

Examine changes in legislation and regulatory standards that influence the practice of case management.
Relate utilization review to discharge planning, reimbursement and key outcome measurements.
Describe the case management role in core measures as well as emerging roles in case management.
Discuss case presentations of difficult to discharge patients and examples of patient throughput/patient flow.
Identify strategies to manage chronic disease across the continuum.
Recognize the value of evidence-based medicine and its implications for case review appropriateness.
